U.S.A. Visa Requirements
For those doctors interested in attending the IAO Annual Meeting, please read and
consider this information carefully.

Visa Waiver Program
The Visa Waiver Program determines which passport holders do not need a visa to
enter the United States of America and the requirements of each individual's
visit. Additionally, this program outlines any exceptions for individuals
who may still need a U.S. visa even if they are from a Visa Waiver Country.

Applying for a U.S. visa
If you need a visa for entry into the U.S.A., please contact your local U.S. embassy or consulate.

U.S. Visa Invitation Letters
The IAO recommends you organize your visa request from your local U.S. Embassy
or Consulate as soon as possible because delays can occur.
If you
would like to request an invitation letter to the 2011 IAO Annual Meeting in
Scottsdale, Arizona U.S.A. on April 7-10, 2011 please contact Jenn (jennba@iaortho.org)
at IAO Headquarters to request your invitation letter. Your request must include your full
mailing address and your name exactly
as it appears on your passport.
